Template Email to Grantees Requesting Design Survey Interview

Subject: National PREP Evaluation

We are writing on behalf of the Administration for Children and Families (ACF) within the U.S. Department of Health and Human Services. As you may be aware, Mathematica is conducting an evaluation of the PREP program for ACF which includes multiple components. The evaluation will document how state PREP programs are designed and implemented, measure and report on program performance, and, within a small number of selected sites, examine the impacts and implementation of the PREP programs. These three evaluation components are described more fully as the Design and Implementation Study, the Performance Analysis Study, and the Impact and Implementation Study in the attached evaluation overview.

We are currently in the process of conducting interviews with state officials who oversee their state’s PREP program. These interviews will contribute to the Design and Implementation Study. The interviews will help us to learn more about what PREP programming decisions have been made, the reasons why some key decisions were made, and the intentions for delivering the programs in each state.

The interview will not last more than one hour, and we would like to complete the interview with you within the next two weeks. Could you please recommend up to five days and times (EST) between [DATE] and [DATE] that you would be available for the interview?

We understand that [STATE] may have several officials overseeing PREP program design and implementation decisions. If you would like to designate another state official to complete this interview on your behalf, could you please provide us with their name and contact information?
