• F_P= Participating Facilities

• F_C= Cooperating Facilities

• F_T=Total Facilities in Sampling Frame

Raw and Adjusted Patient Response Rates

• I= All complete Interviews

• P= All partially complete interviews

• R= Refusals

• NC= Non-Contacts

• U= Cases of unknown eligibility

• e= Estimated Eligibility Rate

• e*U= Expected number of eligible individuals among those cases with unknown eligibility status
