Field Trip Review

Dear (teacher’s name),
We were so excited to have you and your students participate in our ranger-led field trip today! In this
package you have two sets of forms. One form is the Teacher Evaluation Form and the other is the Student
Evaluation Form. It should take you no more than 2 minutes to complete the teacher’s form; and the
students’ form should take about 5 minutes for them to complete. Please administer forms only to the
students who attended the field trip.
We have provided you with a self-addressed, postage paid envelope that you should use to return the forms
to our office. Please feel free to contact me if you need additional forms. I will be happy to send you
replacement forms. You can contact me at [email protected] (e-mail). I hope to receive the completed
forms from you soon.
