Field Trip Review
Dear (teacher’s name),
We were so excited to have you and your students participate in our ranger-led field trip a few weeks ago. So
far we haven’t received the evaluation forms we provided you at the end of the field trip. Please take a few
moments to complete the forms and have your students (only the students who attended the field trip) finish
the student forms. The teacher form should take you no more than 2 minutes to complete; and the students’
form should take about 5 minutes for them to complete.
Please use the self-addressed, postage paid envelope we provided to return the forms to our office. If you
need additional forms please do not hesitate to contact me as soon as possible and I will be happy to send you
replacement forms. You can contact me at [email protected] (e-mail). I hope to receive the completed
forms from you soon.
