All questions concerning production and purchases refer to the period beginning July 1, 2008 and ending June 30, 2009. Please complete this questionnaire and return it by June 9, 2009. A postage-paid envelope is enclosed for your convenience or you may fax the report to 1-808-973-2909.

Response to this survey is voluntary and not required by law. However, your cooperation is extremely important in order to make accurate macadamia nut estimates for Hawaii. Instructions: The questions below refer to macadamia nuts harvested during the 2008-2009 season which

started July 1, 2008 and ends June 30, 2009. Please make an estimate for the remaining weeks of June.

Exclude husking losses from gross pounds. If nuts are delivered wet in-husk,
