No Material or Non-substantive Change Request Justification



Passport Services submitted a no material or non-substantive change request justification and received OMB approval on July 20, 2012. No questions were altered/changed, there were only page formatting changes. The following changes were made:

  • Passport Services significantly enlarged the Acceptance Facility portion of the DS-11 in order to make it easier to scan and input the applicant’s identification information into our systems. The changed section begins on Page 1 at the STOP! CONTINUE TO PAGE 2 line, and continues on the 2nd page (the enlarged section forced a carryover to page 2).

  • Passport Services has added their address to the PRA Statement for public comment on burden issues. Previously the address indicated the Office of Directives Management. This change is to ensure burden comments go to the correct Office.



Passport Services is now requesting OMB rescind the July 20, 2012 approval and reinstate the DS-11 that was previously approved on February 23, 2011. Due to significant coordination efforts needed with government contract partners, software development, etc., Passport Services cannot process the revised DS-11 form that was approved on July 20, 2012 for approximately six months.
