The Application for a U.S. Passport
(Form DS-11) is used by citizens and non-citizen nationals
(hereinafter, collectively referred to as "nationals") of the
United States who are applying for a U.S. passport (a travel
document attesting to one's identity and U.S. nationality), and who
must apply in person.
