Abstract: RI-38-128 is used to give recent retirees the opportunity to sign up or to waive Direct Deposit of their payments from OPM. This form is needed only if the separating agency did not give the retiring employee this election opportunity. This form has been revised to bring the program office name up-to-date and change the number of fields for the account number.
