A Message from the Director, U.S. Census Bureau:
Your household has been selected to participate in a survey on housing conditions that the
U.S. Census Bureau is conducting for the city of New York under the authority of Title 13, of
the United States Code, Section 8b. The city will use the survey results to design programs
and policies to improve conditions throughout the city.
A Census Bureau field representative, displaying an official identification card, will visit
selected households very soon. Participation is voluntary; however, your household is part of
a scientifically selected random sample, so your answers will represent hundreds of similar
households throughout the city, in addition to your own. For this reason, your participation is
extremely important and will help to ensure the accuracy and completeness of the final results.
Title 13 of the United States Code requires that all information obtained by the field
representative be kept strictly confidential. It can be seen only by people sworn to uphold the
confidentiality of Census Bureau information. The Census Bureau will use the information for
statistical purposes only and will not release any information that identifies an individual
household or person to anyone – no government agency, no private organization, no
individual. This survey was approved by the Office of Management and Budget and was
given Approval No. 0607-XXXX, which will appear on each survey form. This collection will
expire on XXX XX, 2014.
The survey interview will take approximately 30 minutes. If you have any comments regarding
