Medical Loss Ratio (MLR) Report for Medicare Advantage (MA) Plans and Prescription
Drug Plans (PDP)
The following revisions are in response to public comments received during the 60-day public
comment period. A 60-day Federal Register notice was published on July 5, 2013, Vol. 78, No. 129,
pg 40482-40484. Collectively, these revisions do not result in a change to the estimated burden.
1) Worksheet 1, Section 2, Line 1: Revenue
Reduce the number of data entry fields by consolidating/removing/revising several fields, as
follows:
 Former Lines 1.1a, 1.1b, and 1.1c have been consolidated into Line 1.1a
 Former Lines 1.1d and 1.1e have been consolidated into Line 1.1b
 Former Lines 1.1f and 1.1g have been consolidated into Lines 1.1a and 1.1b
 Former Lines 1.2, 1.3a, and 1.3c have been consolidated into Line 1.2
 Former lines 1.8a, 1.9, 1.9a, and 1.9b have been removed
Renumber Lines 1.3b and 1.3c into Lines 1.3 and 1.4
Add new Line 1.5 for MSA Enrollee Deposit
Renumber Former Lines 1.4 through 1.8 into Lines 1.6 through 1.10
Revise/clarify labeling of Lines 1.1, 1.3, and 1.4
2) Worksheet 1, Section 2, Line 2: Claims
Reduce the number of data entry fields by consolidating/removing/revising several fields, as
follows:
 Former Lines 2.1a and 2.1b have been consolidated into Line 2.1
 Former Lines 2.2a and 2.2b have been consolidated into Lines 2.2
 Former Line 2.3 has been revised into Lines 2.3a and 2.3b
 Former Line 2.4 has been removed
 Former Line 2.5 has been revised to Line 2.8b
 Former lines 2.9, 2.9a, and 2.9b have been removed
