We have not received your input on CDC’s Infectious Diseases (ID) Reference Laboratory Services as yet. The assessment is designed to take 5-10 minutes to complete with 10 short checkbox questions with comment boxes to provide additional details. You may designate another person familiar with ordering testing from CDC and receiving reports to answer the questions. The assessment will officially close on _(Insert date)_. We would appreciate hearing about your satisfaction with the recent process improvements made to our CDC’s ID Reference Lab services and service areas where we can improve to meet the needs of your Public Health Laboratory.
The responses from 2012 guided several process improvement projects, including the CDC Infectious Diseases Test Directory and electronic reporting of test results by CDC’s ID laboratories with StarLims capability. We need your feedback to know if these changes are working effectively and where other process improvements are needed for our Reference Laboratory Services.
