The Examination Questionnaire provides
the OCC with information needed to evaluate the effectiveness of
the examination process and agency communications. The OCC uses the
information to identify problems or trends that may impair
examination effectiveness and to improve its service to national
banks. The OCC is adding an Exit Survey, which will help the OCC
understand the underlying reasons why banks and thrifts decide to
leave the system. This non-material change involves requesting
certain existing respondents to answer an existing question that
they had previously been directed to omit.

the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
