Consent: This study is sponsored by the U.S. Department of Labor, Bureau of Labor Statistics. This voluntary study is being conducted under OMB Control Number 125-0088 (Expiration: August 31, 2017).  Without this currently approved number we could not conduct this survey.  You may decline to answer specific questions. You also may end the interview at any point. All personnel associated with this study have signed a legal document in which they pledge to protect the confidentiality of the information that survey participants provide.


Instruction: We hope that you will complete all of the questions in one sitting. To go through these questions should only take less than 10 minutes.
