Abstract: This Supplement to the National Ambulatory Medical Care Survey (NAMCS) will provided information about the current state of primary care management policies in physician practices in the United States. Policies of interest are those aimed at management of adult patients with high blood pressure, high cholesterol, or diabetes. CDC and its partners want to better support primary care physicians and their medical practices in reducing mortality and morbidity due to these chronic conditions.
