Abstract: SSA uses Form SSA-3 to collect information to determine if a spouse claimant has the necessary relationship to the number holder (i.e., the worker) to qualify for Old Age and Survivors Disability Insurance (OASDI) benefits. The respondents are applicants for spouse's OASDI benefits. We are making form SSA-3 a fillable form.
