Cancer Control P.L.A.N.E.T. Evaluation Survey
The Cancer Control P.L.A.N.E.T. is a web portal that provides access to data and resources that can
be used to design, implement, and evaluate evidence-based cancer control programs. We would
like your feedback on how the site can better meet your needs.
The survey takes approximately 10-12 minutes to complete. It will ask you about your experience
with Cancer Control P.L.A.N.E.T., and then a few questions about you .

You are invited to participate in an online survey about the National Cancer Institute Cancer Control
P.L.A.N.E.T. web portal. This survey asks about your current use of, and satisfaction with, the portal. It
should take approximately 10-12 minutes to complete.
Your participation in this survey is voluntary. You may refuse to take part in the evaluation or exit the survey
at any time without penalty. You are free to decline to answer any particular question you do not wish to
answer for any reason.
BENEFITS/RISKS
You will receive no direct benefits from participating in this evaluation. However, your responses may help
us learn more about how people use Cancer Control P.L.A.N.E.T., and how the portal can be improved.
There are no foreseeable risks involved in participating in this evaluation other than those encountered in
day-to-day life.
CONFIDENTIALITY
Your survey answers will be sent to a link at SurveyMonkey.com where data will be stored in a password
protected electronic format. Your responses will be confidential and not be shared with anyone outside of
the evaluation team.
At the end of the survey you will be asked if you are interested in participating in an additional interview by
phone to provide more detailed feedback about specific changes you would like to see made to Cancer
Control P.L.A.N.E.T. If you choose to provide contact information (email address), your survey responses
may no longer be anonymous to the researcher. However, no names or identifying information will be
included in any publications or presentations based on these data, and your responses to this survey will
remain confidential.
