Parent Consent Forms. Please distribute parent consent forms to each child’s parent/caregiver immediately. This form gives permission for researchers to conduct a survey with children under your care during our visit. Children do not need permission to participate in the activities, only to participate in the survey. Place all signed consent forms and questionnaires in the folder marked “Signed Child Consent Forms”.

The Food Explorers: Tasting the World! Family Cookbook. The Family Cookbook contains recipes for families to try at home. Please give a cookbook to each parent with the consent form.

Food Explorers: Tasting the World! Nutrition Education Guide. This packet contains the recipes and activities you will be trying out with children under your care. Please complete 8 activities before [DATE]. You have been assigned 6 activities and recipes listed above and can choose any 2 additional activities and recipes. Do not begin activities until the research team has conducted the first round of surveys with children.

Provider Journal. As you try each activity and recipe, please complete the provider journal. Instructions for how to complete the journal are on the journal’s cover page.

Taste Test Ballot and Records Sheets. Please administer a taste test with 5 children immediately after completing each recipe. Instructions on how to do the taste test can be found on the cover page of the taste test packet.

Food Explorers: Tasting the World Poster. Please hang the poster in a visible spot in your child care.

A member of the research team will go over all of this information with you when they visit your site on [DATE] to conduct the student pre-test survey.
