Abstract: Information provided is the application for HUD/FHA multifamily mortgage insurance. The information from sponsors and general contractors, and submitted by a HUD-approved mortgagee, is needed to determine project feasibility, mortgagor/contractor acceptability, and construction cost. Documentation from operators/managers of health care facilities is also required as part of the application for firm commitment for mortgage insurance. Project owners/sponsors may apply for permission to commence construction prior to initial endorsement.

Short Statement: This is a revision of a currently approved collection. The supplemental Forms HUD-92013-D and Riders HUD-92466-R5 and HUD-92466-R6 will assist in the processing of the FHA mortgage insurance application to determine the appropriate mortgage insurance premium. HUD announced a change to the premium rate structure that will simplify the premiums charged into four rate categories: Market Rate, Broadly Affordable, Affordable, and Green/Energy Efficient Housing. The new rate structure was effective April 1, 2016 with the announcement in the Federal Register Vol. 81, No. 62 18473 dated Marche 31, 2016.
