Report” and FNS-742a “Local Educational Agency Second Review of Applications
Report” to the Food Program Reporting System (FPRS) OMB Control Number 05840594, expiration Date 6/30/17
The information collection (ICR) burden associated with the FNS form titled “School
Food Authority (SFA) Verification Collection Report” and the FNS form titled “Local
Educational Agency Second Review of Applications Report” is approved under OMB
#0584-0026 with an expiration date of 04/30/2016. Since its last approval, FNS has
consolidated certain programmatic and financial data reporting requirements approved
by the Office of Management and Budget under the Food Programs Reporting System
(FPRS), an electronic reporting system. These forms are being included in this
consolidation.
FNS is requesting a no material/non-substantive change to merge the reporting burden
from OMB Control Number 0584-0026 for the FNS-742 and the FNS-742a (which has
been renamed FNS-874 to comply with FPRS form numbering structure) forms to OMB
Control Number 0584-0594. The burden estimates for these forms remain the same and
there are no new data elements introduced. The recordkeeping burden will remain in
OMB Control Number 0584-0026. The merge will increase the current burden inventory
for OMB Control Number 0584-0594 as follows:
