Background

The SSA-4588 form, (You Can Make Your Payment by Credit Card), provides debtors the option to pay their Title II and Title XVI debts by MasterCard, VISA, Discover, American Express, or Diner’s Club credit cards. The form gives the debtor the option to make a one-time payment or establish recurring monthly payments. We accept the Diners Club Card only when it processes as a MasterCard or Discover transaction. Acceptable Diners Club Cards should have the MasterCard or Discover Card logo present on the card and meet the numerical requirements (See chart GN 02210.205B.1).

There are six versions of the SSA-4588, one form for each PSC; it contains the PSC‑specific telephone number and hours of operation information.

Revision to the SSA-4588-OP6:

• Change #1: On Page 1 Question 1, under “Then Do This…,” we corrected office hours from 6:15AM-5:00PM to 8:00AM-4:00PM.

Justification #1: We made the change to reflect correct office hours.

• Change #2: On Page 3 under “Terms and Conditions of Authorization,” we corrected office hours from 6:15AM-5:00PM to 8:00AM-4:00PM.

Justification #2: We made the change to reflect correct office hours.

SSA will implement the changes to the SSA-4588-OP6 upon OMB approval.

This action does not affect the public reporting burden.
