Thank you in advance for completing this brief survey. We value your input. The answers you
provide will be used by the NSF INCLUDES Coordination Hub to help inform our work to
strengthen the Network. This survey is not anonymous, but we will not associate your personal
or organizational name to specific responses in any public documents without first getting your
permission to do so. Your responses will be kept confidential unless and until you provide such
permission. The survey will take about 15 minutes to complete.
