The time required for the SFA Director to complete this information collection is estimated to average 10 minutes per response, including the time to review instructions, search existing data sources, gather and maintain the data needed, and complete and review the collection of information.

3. Please provide a date and time within the next two weeks in which the data collector can conduct a brief in-person data collection visit. As a reminder, the data collector will only need access to the records for approximately 30 minutes while at the SFA. You can also call us at the study toll free number (listed below) to schedule the visit for a day that is convenient for you.
