Conditionally Approved Status in COLAs Online – Follow-Up User Survey



Survey questions for COLA applications in Conditionally Approved status that timed out or had changes declined.

Interviewers will contact respondents by telephone. Interviewers will introduce themselves and state that they are conducting a short survey about industry member use and acceptance of the Conditionally Approved status in COLAs Online.

The questions to be asked are:

(1) Are you directly employed by the industry member that produces the alcohol beverage on the label, or are you a third-party representative submitting on behalf of the industry member? [Interviewer will code as appropriate.]

(2) Why did you decline the proposed changes? [Or: Why did you let the proposed changes time out]?

Interviewer will code responses into one of the following categories:

  • I wanted to make the changes myself.

  • I didn’t agree with the proposed changes.

  • I didn’t understand how the status works.

  • I forgot to take action before the 7-day deadline.

  • Other [Interviewer will make notes].

(3) Do you receive the TTB newsletter? [Yes/No]

(3a) (If yes to # 3) Do you recall seeing an article about the Conditionally Approved status? [Yes/No]

(4) Did you participate in either of the webinars on the Conditionally Approved status? [Yes/No]

(5) Do you have any feedback for us on the Conditionally Approved status? [Interviewer will make notes.]
