Information regarding a parent, an incapacitated child over age 21, a full time student 21-22 years of age or a ward of a court is provided by the military member or by another individual who may be a member of the public.

Short Statement: This is a reinstatement with change to an expired collection. Number of respondents was reevaluated and a better approach to identifying the numbers was used, causing a decrease in forms submitted. Also, one of the forms that comprised the previous OMB submission is no longer being used.
