Summary of Changes Made to 9906 Charts

General

• LHCA and Intermediary/SHFA/MSOs now have separate sets of application forms.

  • The LHCA set, HUD-9906-L, is entitled, “Local Housing Counseling Agency (LHCA) Application.” Charts for LHCA applicants are now designated A1, B1, C1, D1, E1, F1, and G1.

  • The Intermediary/SHFA/MSO set, HUD-9906-P, is entitled, “Intermediary, State Housing Finance Agency, and Multi-State Organization Application.” Charts for Intermediary/SHFA/MSO applicants are now designated A2, B2, C2, D2, E2, F2, and G2.

• Updated dates to reflect grant period

Chart A1

• Field E changed from “Preferred Sustainable Communities / Promise Zones” to “Promise Zones Housing Counselors Received Emergency Preparedness or Disaster Recovery Training”

• Field K changed from “Number of HUD HECM Roster Counselors (if applicable)” to “Number of HUD HECM ROSTER reverse mortgage counselors (if applicable)”

• Field N changed from “Require Testing/Certification for Counselors” to “HUD-certified Housing Counselors On Staff”

• Field W changed from “Geographically Isolated Agency” to “Sole Agency Providing Housing Counseling Services in Target Area”

• Field A1 now includes a text box; if Applicant marks “Uses Other Methods of Evaluating Program Service,” Applicant must also describe these methods in the provided text box.

Chart B1

• Chart B1 streamlined to remove fields not applicable to LHCA applicants

Chart C1

• Chart C1 streamlined to remove fields not applicable to LHCA applicants

• “(Must Provide Explanation of Other Expenses in Narrative)” has been added to Line 13, Total Other Direct costs

• “Other (Must Provide Explanation of Other Expenses in Narrative” has been removed

HUD-9906-P (Intermediary/SHFA/MSO Application)

Chart A2

• Chart A2 form streamlined to ask for basic characteristics of the Intermediary/SHFA/MSO applicant

• Intermediary/SHFA/MSO must complete and attach the Chart A2 xls attachment with their branch and/or sub-grantee information, and enter the totals from the attachment into the Chart A2 “Totals from Chart A2 Attachment” form.

  • Chart A2 was decoupled from pdf of 9906-P at suggestion of grants.gov team
