Defense Federal Acquisition
Regulation Supplement (DFARS) Part 237, Service Contracting,
associated DFARS Clauses at DFARS 252.237, DD Form 2062, and DD
Form 2063
(1) 252-237-7000 is used to provide
information that enables verification that the apparently
successful offeror for audit services is properly licensed in the
state or other political jurisdiction where it operates its
professional practice; (2) 252.237-7011, the DD Form 2062 and DD
Form 2063 are used to ensure the mortuary contractor has properly
prepared the body; (3) 252.237-7024/252.237-7023 require offerors
to submit a written plan describing how it will continue to perform
essential contractor service and contractors to maintain and update
the plan as necessary.
In accordance with Title
5, Code of Federal Regulations (CFR) Act, Part 1320.13, “Emergency
Processing,” the Department of Defense requests emergency
processing and approval of a revision to a public information
collection. The collection of concern is 0704-0231, “Defense
Acquisition Regulations System (DFARS) Service Contracting, Record
of Preparation,” which currently includes DD Form 2063, “Record of
Preparation and Disposition of Remains (Contracted Mortuary
Facility).” The Department of Defense seeks to revise the
collection to also include DD Form 2062, “Record of Preparation and
Disposition of Remains (DoD Mortuary Facility).” This form records
services performed by a licensed mortician in the reprocessing of
remains of the decendent. The nature of DD Form 2062 and the impact
it has on service members and their families elevates the urgency
of clearing this instrument with its counterpart form (DD Form
2063). Complying with the normal clearance procedures would delay
the approval of this form and have a negative impact on our service
members and their families who may already be experiencing the
stress and grief of losing a loved one.
This IC has been revised to
include the burden associated with the DD Form 2062, Record of
Preparation and Disposition of Remains (DoD Mortuary
Facility).

with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
