This submission is being made to OMB
as a non-substantive change request to an existing information
collection pursuant to 44 U.S.C. § 3507. This submission seeks to
fill in the application deadline date of Monday, June 4, 2018 on
the accompanying instructions to FCC Form 5625 for OMB Control
Number 3060-1241.On May 18, 2018, the Commission announced the
application deadline date for this submission. In this
non-substantive change, we are updating the application deadline
date on the instructions for FCC Form 5625. This submission is
being made to OMB as a non-substantive change request to an
existing information collection pursuant to 44 U.S.C. § 3507. This
submission seeks to change the commercial overnight mailing address
from 9300 East Hampton Drive, Capitol Heights, MD 20743 to 9050
Junction Drive, Annapolis Junction, MD, 20701 on the accompanying
instructions to FCC Form 5625 for OMB Control Number 3060-1241.
Under this information collection, the Commission proposes to
collect information to determine whether applicants that were
selected in coordination with New York's New NY Broadband Program
are technically and financially capable of meeting the terms and
conditions of Connect America Phase II support.
