Minor Revisions to the Collection Instruments

We are making a non-substantive changes within a year of OMB approval to update the Privacy Act Statement. SSA’s Office of General Counsel conducted a systematic review of the Privacy Act Statement, and revised the language after we obtained OMB approval. We are making the changes now instead of waiting for renewal to ensure all applications have the correct Privacy Act Statement

SSA is making the following revisions:

  • Change #1: We are revising the Privacy Act Statements on the Internet & Intranet Registration for Appointed Representative Screens.


Justification #1: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statements on these Screen Packages.

  • Change #2: We are revising the Privacy Act Statement on the N8NN Telephone Call Flow Screens.

    Justification #2: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on this Screen Package.

  • Change #3: We are revising the Privacy Act Statement on the Preface to the Telephone Script Document.

    Justification #3: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on this Document.
