Supporting Statement Part A

• The timelines for survey follow-up in Sections A.2 and A.3 are inconsistent (7-8 weeks vs. 6 weeks).

• The calculation of electronic responses in Section A.3 appears incorrect if all surveys are expected to be submitted electronically.

• The average number of responses per non-respondent in Question 12A cannot be right. You are reaching out the CN Directors at most 7 times and SFA Directors a max of 8 times. The average number of responses can’t be larger than the number of things you are asking them to respond to.

• Need clarification regarding how the pretest participants completed the pretest surveys. Did they mail them?
