The DD Form 2826 is used to report on
the administration of the funds received on behalf of a mentally
incompetent member of the uniformed services. A designated Trustee
is required to report the dates, amounts and reasons for payments
made. Failure to report the proper information may result in the
termination of the Trusteeship.
