This is a request for revision of the Fellowship Management System (FMS) information collection to continue to collect data under the approved OMB No. 0920-0765. CDC uses FMS to collect, process, and manage data from non-federal applicants seeking training or public health support services through CDC fellowships. FMS is used to electronically submit fellowship applications and fellowship host site proposals, and to maintain fellowship alumni directories on-line.

CDC estimates a net increase of 225 annualized burden hours. The number of fellowship applicants is expected to increase, but burden per response for most applications will decrease due to use of a new, more efficient web-based system. The number of fellowship alumni who participate in the alumni directory is expected to increase. In addition, the new platform includes 2 new functions that will increase the total number of respondents and burden hours. The new system will electronically collect 1. references for fellowship applicants, and 2. data on fellows' activities during the period of the fellowship..
