A Message from the Acting Director, U.S. Census Bureau:
We are requesng your parcipaon with the Quarterly Financial Report. The U.S. Census
Bureau conducts this survey in order to provide vital stascs describing current business
conditions of the U.S. economy. Survey results are used to prepare national measures of
corporate profits, including estimates of the Gross Domestic Income, and to formulate fiscal
and monetary policy. Your company’s participation is critical to this effort.
Use the enclosed form as a worksheet prior to reporting online, or return the completed form
in the enclosed envelope. Online reporting is easy, reduces costs, and decreases processing
times. Website and login information can be found on the front page of the form.
YOUR RESPONSE IS REQUIRED BY LAW and will be kept strictly CONFIDENTIAL. Information
about the authority, confidentiality, and burden of this data collecon can be found on the
back of this letter.

This collection has been approved by the Office of Management and Budget (OMB). The
eight-digit OMB approval number is 0607-0432 and appears in the top right corner of the
paper survey form or each online reporting screen. Without this approval, we could not
conduct this survey.
Authority and Confidenality
Title 13, United States Code, Section 91, authorizes this collection. Sections 224 and 225
require your response. The U.S. Census Bureau is required by Section 9 of the same law to
keep your information confidential and can use your responses only to produce statistics. The
Census Bureau is not permied to publicly release your responses in a way that could idenfy
your business, organization, or institution. Per the Federal Cybersecurity Enhancement Act of
2015, your data are protected from cybersecurity risks through screening of the systems that
transmit your data.
Burden Esmate Statement
We estimate the Quarterly Financial Report will take an average of 1.2 to 3 hours to complete.
Factors such as company size, complexity, and acvity will affect your actual me to complete
the survey. This estimate includes the time to review instructions, search existing data sources,
gather and maintain the data needed, and complete and review the survey.
