Pursuant to 42 U.S.C. § 5422 (g) (section 6 23(g) of the National Manufactured Housing Construction and Safety Standards Act of 1974) HUD will implement a dispute resolution program in e ach State that does not have a program meeting the requirements of 42 U.S.C. § 5422(c)(12). This Dispute Resolution Certification Form will be used for states to self-certify the adequacy of the state’s dispute resolution program and f or HUD to review that self-certification. Acceptance of your state’s program will be determined by reviewing whether the response to Part II of this form complies with the requirements of 24 C.F.R. 3 288.205. You r answers to the following g questions are requested for a proper review.


42 U.S.C. § 5422(g) (section 623(g) of the National Manufactured Housing Construction and Safety Standards Act of 1974) authorizes HUD to implement a dispute resolution program in each State that does not have a program meeting the requirements of42 U.S.C. § 5422(c)(12). Your answers to the following questions are necessary for a proper evaluation of your dispute. The respondents are homeowners, installers, retailers, and manufacturers of manufactured housing. HUD does not pledge assurance of confidentiality to respondents. HUD generally discloses this data only in response to a Freedom of Information request.

HUD collects information in accordance with 42 U.S.C. § 5422(g) (section 623(g) of the National Manufactured Housing Construction and Safety Standards Act of 1974). This Dispute Resolution Certification Form will be used for states to self-certify the adequacy of the state’s dispute resolution program and for HUD to review that self-certification. Acceptance of your state’s program will be determined by reviewing whether the response to Part II of this form complies with the requirements of 24 C.F.R. 3 288.205. All information provided is voluntary. Failure to provide information could delay the processing of the certification. HUD generally discloses this data only in response to a Freedom of Information request or to a state, local agency, and the agency and person who are involved and/or all businesses that are affected by the dispute.


This language at the top of the form was modified to include all of the require elements for a Paperwork Reduction Act burden statement and to make the form 508 compliance.
