Privacy Act Statement
Authority: Public Law (PL) 91-357; 5 U.S.C. 1302, 2951, 3301, 3372, 4118, 8347, and Executive Orders 9397, as amended by 13478, 9830, and 12107.
Purpose: The information collected is required to identify persons interested in participating in a government volunteer program, and for all necessary purposes for managing the volunteer program, including tort claims and injury compensation. This is an interagency effort between the Department of the Interior and Department of Agriculture, U.S. Department of Commerce and U.S. Department of Defense.
Routine Uses: In addition to those disclosures generally permitted under 5 U.S.C.552a(b) of the Privacy Act, records or information contained in this system may be disclosed outside the agencies participating in this program as a routine use pursuant to 5 U.S.C. 552a(b)(3) when the disclosure is compatible with the purpose for which the records were compiled as described under the DOI-05, Interior Volunteer Services File System and OPM/GOVT-1, General Personnel Records.
Disclosure: Voluntary, however, failure to provide the requested information may impede individual from participating in a government volunteer program.

Collection and use is covered by Privacy Act System of Records DOI-05 and OPM/GOVT-1, and is consistent with the provisions of 5 USC 552a (Privacy Act of 1974), which authorizes acceptance of the information requested on this form. The information is used to identify persons interested in participating in a government volunteer program and to manage the volunteer program, including tort claims and injury compensation. Furnishing this information is voluntary, however, failure to provide the requested information will prevent program participation.
