(E) Epidemiology (During reporting period). Section E should only be completed if reporting state identifies CWD suspect, exposed or positive animals/ herds.


Definitions:



CWD-Positive Animal: An animal that has had a diagnosis of CWD established through official
confirmatory CWD testing conducted by the National Veterinary Services Laboratories (NVSL).



CWD-Positive Herd: A herd in which a CWD-positive animal resided at the time it was diagnosed and
which has not been released from quarantine.



Trace-Forward Herd: A herd that has received exposed animals from a CWD-positive herd within
5 years before the diagnosis of CWD in the positive herd or from the identified point of entry of CWD into
the positive herd.




Traceback Herd: A herd in which a CWD-positive animal formerly resided.


CWD-Exposed Animal: An animal that is part of a CWD-positive herd, or that has been exposed to a CWD-positive animal or contaminated premises within the previous 5 years.



CWD-Exposed Herd: A herd in which a CWD-positive animal has resided within 5 years before that
animal’s diagnosis as CWD-positive, as determined by an APHIS employee or State official.

Note: All data in this section includes HCP and non-HCP herds during performance period stated in (A)(1)












(B) 1-5 is a summary that includes all species of cervids for HCP enrolled and non-enrolled herds. This will calculate and self populate from other data entered.












(B) 6-10 includes HCP enrolled and non-HCP enrolled elk herds (elk, red deer, sika deer, and hybrids)












(B) 11-13 includes HCP enrolled deer herds (herd with 100% CWD susceptible deer; WTD, Mule deer or Black-tail deer)












(B) 14-15 includes non HCP enrolled deer herds (herd with 100% deer that can be susceptible and non susceptible , WTD, mule deer, black tail, fallow or axis)












(B) 16-18 includes herds with deer and elk; CWD susceptible species; herds not included in B8 or B13












(B) 19-20 includes herds with deer and elk; CWD susceptible and nonsusceptible species; not included in B9, B10, B14, B15












(B) 21, 23, 25, 27, 29, 31, 33, and 35 will populate automatically.












(B) 22, 24, 26, 28, 30, 32, 34, and 36 Range: the smallest herd to the largest herd size (e.g. 20-450)


























Section C: Only applies to CWD HCP enrolled herds unless otherwise specified












Note: Herd inspections are required annually. Physical inventories required at least every 3 years.












Note: Physical inventories require that individual animals and their IDs be reconciled with inventory records












(C) 1: Total annual inspections conducted during reporting period by State/ Federal personnel or accredited veterinarians.












(C) 2: Total physical inventories conducted during reporting period by State/ Federal personnel or accredited veterinarians.

(D) 1 Includes all farmed/ captive cervids in HCP enrolled herds CWD tested for any reason.












(D) 2 Includes all farmed/ captive cervids not in HCP enrolled herds CWD tested for any reason.












(D) 3 will self-populate as data is entered












(D) 4 Include animals in HCP enrolled herds that have died from trauma, disease, unknown, etc. (exclude hunted or slaughtered animals).












(D) 5 Include animals in HCP enrolled herds that are slaughtered at commercial or custom facilities.












(D) 6 Include animals that are harvested in HCP enrolled herds by hunting or onsite slaughter.












(D) 7 Include animals that are identified as suspects and euthanized or slaughtered.












(D) 17,18 and 21: species not CWD susceptible under the CFR however they may be in a HCP herd and they may have been tested so include that data

Section E:












Note: Section E should only be completed if your state has identified CWD suspect, exposed or positive animals/ herds












(E) 1-9: Include data from all captive/ farmed herds (not limited to enrolled herds).












(E) 10-32: Include data from all captive/ farmed herds (not limited to enrolled herds). Trace forward exposed herds have received CWD exposed animals. Each animal traced is counted as an investigation.

(E)7, (E)18 and (E)29: Examples could include investigation pending, owners declined to signed herd plan, etc.












(E)8, (E)19 and (E)30: Herd plans are completed when all conditions have been satisfied.












(E)24: Potential source herds would include all herds that a CWD positive animal was in for the five years preceding diagnosis.
