Subject:	NonSubstantive Change Request – Healthy Marriage and Responsible Fatherhood Performance Measures and Additional Data Collection (OMB #0970-0566) 


This memo requests approval of nonsubstantive changes to the approved information collection, Healthy Marriage and Responsible Fatherhood Performance Measures and Additional Data Collection (OMB #0970-0566). 

Background
The Office of Management and Budget (OMB) approved the Healthy Marriage and Responsible Fatherhood (HMRF) Performance Measures and Additional Data Collection in July 2026. This information collection package supports the collection and reporting of performance measures by the 2025 cohort of Administration for Children and Families (ACF)-funded HMRF grant recipients. Grant recipients can complete data entry via the Information, Family Outcomes, Reporting, and Management (nFORM) system. Grant recipients began data collection on July 13, 2026, and initial performance progress reports are due in October 2026.

Through additional review, we have identified minor changes to better align the instruments with ACF’s notices of funding opportunities for HMRF grants and to clarify instructions for respondents. The proposed changes do not affect respondent burden, and the purpose and use of the information collection remains the same. Details about the changes are provided below.

Overview of Requested Changes
    1. nFORM application form (C2) screen (OMB instrument 1a. nFORM application form). The original OMB package indicated that the fields related to screening for intimate partner or teen dating violence would be made optional but did not specify that the fields would be removed. This change request removes those fields from the screen shots. Please see updated Instrument 1a. nFORM Application Form. 

    2. nFORM Client profile (C3) screen (included in OMB Instrument 3. nFORM Service Receipt). The statement indicating whether or not the client has been screened for intimate partner violence or teen dating violence is being removed from the Client Information box on the Client Profile. This change was documented in the original OMB package, but a new screen shot was not included. Please see updated Instrument 3. nFORM Service Receipt.   

    3. Service contact (C7) and referral (C12) screens (included in OMB Instrument 3. nFORM Service Receipt). A new optional field will be added to the service contact and referral menus on screens C7 and C12 so that grant staff can check whether an assessment for domestic violence/intimate partner violence (DV/IPV) was covered during the service contact or referral. This new field will be labeled “Domestic Violence/Intimate Partner Violence (DV/IPV) Screening”. An existing field on these screens will be relabeled from “Domestic Violence/Intimate Partner Violence” to “Domestic Violence/Intimate Partner Violence (DV/IPV) Services” for clarity. All checkboxes on these two screens are yes/no. The original OMB package indicated that fields would be revised for clarity but did not specify how.     4. Program operations survey (OMB Instrument 2. HMRF Program Operations Survey): The instructions for question A4 are being revised to clarify how to count staff who perform multiple roles for the HMRF grant recipient. This is a new change. Please see updated instrument 2. HMRF Program Operations Survey. 

    5. Performance progress report (OMB Instrument 5. HMRF PPR): 
ACF is embedding the cover sheet from the standard ACF PPR, which has its own OMB approval (OMB #0970-0406), into the HMRF PPR that will be generated by nFORM. This change was documented in the original OMB package. The nFORM-generated PPR will contain all the required items on ACF’s cover sheet but for programming reasons the arrangement of fields will be different. In this nonsubstantive change request, we include a reformatted version of the cover sheet.  

ACF is updating the PPR to better align with its requirements for domestic violence and intimate partner violence services and nonsubstantive changes 1 through 3 described above. Specifically, the narrative prompts in Section B-02.3 of the PPR will be revised, and the content of Table B-02.5 will be revised to use service contact instead of screening data.

Time Sensitivities 
We are aiming to receive OMB approval before grant recipients are required to prepare their initial performance progress reports in October 2026.
