As a reminder, usability testing session scripts are, in general, exempt from PRA review subject to the caveats on p.3 here, but screeners for those sessions must be submitted for PRA review.
PURPOSE OF COLLECTION:
What are you hoping to learn / improve? How do you plan to use what you learn to support service delivery improvement activities? Are there artifacts (user personas, journey maps, digital roadmaps, summary of customer insights to inform service improvements, performance dashboards) the data from this collection will inform?
This collection is intended to assess beneficiary satisfaction with the resolution of complaints entered in the Health Plan Management System (HPMS) Complaints Tracking Module (CTM) handled by CMS Medicare Customer Service, specifically the Medicare Customer Solutions Group (MCSG). Specifically, the survey will measure satisfaction with complaint outcomes, customer service experience, prior engagement with Medicare Advantage Organizations (MAOs) and Part D sponsors, and communication methods used for resolution.
The information collected will be used internally by CMS to identify trends, gaps, and opportunities to improve customer service delivery, complaint resolution processes, and coordination with MAOs and Part D sponsors. Insights from the survey, including structured responses and free-text feedback, will inform internal performance monitoring, customer experience mapping, and process improvement efforts. The data may contribute to internal artifacts such as customer journey maps, service improvement plans, and operational performance dashboards.
This collection is not intended for public release.

4. Who will you collect the information from?
Please describe exactly how you will select the people who you will invite to take the screener or survey, or to participate in the session. Some examples:
• If you are requesting approval for a survey that will appear as a “feedback button” on a website, you can write: “100% of people who visit the website will have the opportunity to take the survey, since the invitation appears on each page of the website.”
• If you are requesting approval for a focus group to conduct discovery phase research, you can write: “of the people who provided their email address to the call center rep, 15% of them will be selected at random to be invited to take part in the focus group.”
These are just examples. The key is you must describe the exact methodology you used to determine who gets the invitation to take part in the screener/survey/session.
Medicare beneficiaries who have submitted a complaint entered in the CTM that was resolved by a CMS caseworker within MCSG, and for whom CMS has a valid email address.
All eligible beneficiaries within this population (approximately 16,000 to 20,000 annually) will receive an invitation to participate in the survey following resolution of their complaint.
Please describe the activity or methodology
Describe the information collection activity – e.g., what happens when a person agrees to participate? Will facilitators or interviewers be used? What’s the format of the interview/focus group?
Following the resolution of a complaint by a CMS caseworker, an automated email will be sent to the beneficiary inviting them to complete a brief customer satisfaction survey.
The survey will consist of five questions and will be administered via Qualtrics. Participation is entirely voluntary, and no follow-up contact will be made for non-response.
The survey is designed to take approximately 2 minutes to complete and will collect both structured (Yes/No and multiple choice) and unstructured (free text) feedback. No interviewers or facilitators will be involved.
• How will you ask a respondent to provide information?
(e.g., after an application is submitted online, the final screen will present the opportunity to provide feedback by presenting a link to a feedback form / an actual feedback form)
After a complaint case is closed, the beneficiary will receive an automatically generated email containing a link to the Qualtrics survey. The respondent can complete the survey by clicking the link and submitting their responses electronically.
• When will the activity happen?
Describe the time frame or number of events that will occur (e.g., “We will conduct focus groups on May 13, 14, 15 of XXXX (year)”; “We plan to conduct customer intercept interviews over the course of the summer of XXXX (year) at the field offices identified in response to #2 based on scheduling logistics concluding by Sept. 10th”, or “This survey will remain on our website in alignment with the timing of the overall clearance.” If you are uncertain as to how long it will take to complete your research, you can write: “Until all participants complete/are interviewed.”)
This survey will be conducted on an ongoing basis, aligned with the closure of beneficiary complaint cases handled by CMS MCSG. Invitations will be sent continuously throughout the duration of the clearance period.

It may be appropriate to compensate participants for reasonable out-of-pocket costs and to recruit those who are hard to reach, such as those with multiple jobs or are caretakers. It is not appropriate to compensate those participating in their work capacity and those who are vulnerable to coercion or undue influence such as children and people with certain mental disabilities.

HPMS collects personally identifiable information on Medicare beneficiaries and complainants in CTM. Specifically, HPMS collects the first name, last name, organization name, address, city, state, zip code, phone number, e-mail address, MBI, and plan member ID. Only the first and last names are required for complainants. None of these fields are required for Medicare beneficiaries. CMS uses these data to investigate MAO and Part D complaints and perform casework activities. SORN number is 09-70-0500 (83 FR 6591).
