Abstract: THIS INFORMATION COLLECTION REQUIREMENT IS NEEDED IN ORDER FOR OSM TO HAVE SUFFICIENT KNOWLEDGE OF EXISTING OPERATORS TO DETERMINE THEIR ELEIGIBILITY FOR ASSISTANCE. THE INFORMATION IS USED BY OSM TO ELIMINATE PROVIDING SERVICES TO OPERATORS WHO ARE NO ELIGIBLE AND FOR WHOM IT WOULD BE IMPRACTICAL TO SEEK REIMBURSEMENT AT A LATER TIME. CATALOG OF FEDERAL DOMESTIC ASSISTANCE NUMBER 15,250.
