amended by ED's memoranda to OMB of 12/9/91 and 12/20/91. ED has
agreed to meet the following terms to improve data quality: -- ED's
program reviewers shall document the existing staff-to-applica
ratio for each completed review. The reviewers shall include an
assessment as to whether the ratio is adequate, to check the
informati on the application based on actual experience as
determined by reviewers. -- ED shall forward any future studies of
staffing ratios received fro NASFAA to OMB. -- Withdrawl rates of
less than 33% shall be entered into the Institutional Data System,
as allowed for by recent system changes. -- ED shall prepare a plan
to collect refund data from participating schools, and shall submit
this plan with the next request for clearanc -- ED shall include
datas on school ownership in the Institutional Dat System.

THE SECRETARY OF EDUCATION MUST
DETERMINE WHETHER POSTSECONDARY EDUCATIONAL INSTITUTIONS MEET THE
STATUTORY AND REGULATORY REQUIREMENT FOR ELIGIBILITY AND
CERTIFICATION TO APPLY FOR FUNDING FOR PROGRAMS AUTHORIZED BY THE
HIGHER EDUCATION ACT OF 1965, AS AMENDED. THE SECRETARY USES THE
INFORMATION COLLECTED ON THIS FORM TO DETERMINE IF AN INSTITUTION
QUALIFIED FOR ELIGIBILITY AND CERTIFICATION.
