Abstract: THIS FORM IS USED TO GATHER INFORMATION ON THE COST AND QUALITY OF FUELS DELIVERED TO ELECTRIC POWER PLANTS. THE RESPONSES ARE USED IN EVALUATION OF INDIVIDUAL UTILITY COSTS AND PRACTICES IN RATE CASES AND IN THE REQUIR PERIODIC REVIEWS TO INSURE EFFICIENT USE OF RESOURCES. THE DATA ARE ALSO USED TO CERTIFY SMALL POWER PRODUCTION FACILITIES AND COGENERATIO PLANTS UNDER THE COMMISSION'S QUALIFYING FACILITIES PROGRAM.
