supports the FCC's use of the Internet and fax-on- demand to
distribute the form, instructions, and related materials.

The FCC form 1240 will be used by
cable operators to file for adjustments to maximum permitted rates
for regulated services to reflect external costs. The FCC form 1240
implements a new optional rate methodology where cable operators
will be permitted to make annual, as opposed to quarterly, rate
changes. It is a filing alternative to the FCC form 1210.
