This collection
is approved on the following conditions as agreed to by NIH: (1)
NIH shall revise the data collection to collect data from every
second call within the five caller categories specified in the
package. This will reduce the burden for this collection by half.
(2) NIH shall delete the word Native before American Indian in the
race categories and send a revised page to OMB to indicate the
change has been made. In addition, this clearance does not cover
the customer service questions which NCI plans to collect thorough
a different survey instrument.

The CIS provides the general public,
cancer patients, families, health professionals, and others with
the latest information on cancer. Essential to providing the best
customer service is the need to collect data about callers and how
they found out about the service. This effort involves asking seven
questions to five categories of callers for an annual total of
approximately 378,165 callers.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
