Justification for Non-Substantial Change

On September 11, 2007 the Commission received OMB approval for information collection 3060-0874. This collection includes FCC Forms 475-B and 2000. After receiving OMB approval for these forms, the Commission has determined that editorial changes are needed to FCC Form 2000¹ to make the form clearer and easier to understand without changing the underlying question.

Therefore, the Commission is submitting a non-substantial change request to OMB for FCC Form 2000 along with the track editorial changes to the form for OMB review and approval.

The following editorial changes have been made to the form:

FCC Form 2000 A

We changed – answer each question that applies to your specific complaint to answer one question that applies to your specific complaint.

FCC Form 2000 C

We added a line for the respondent to supply a fax number, if desired.

We changed – check the appropriate box for your type of complaint to check one box for your type of complaint.

FCC Form 2000 D

We added check one to question number 4.
