To Alter the SORN Originally Cited in OMB package 0920-0765

This change request is to alter the System of Records Number (SORN) that appears in OMB Package 0920-0765 (Part A, Section A10, Page 5). CDC’s Career Development Division (CDD), Office of Workforce Development (OWCD), submitted the OMB package to the CDC OMB office August 23, 2007 and the package was approved by the OMB office February 15, 2008.

When we originally submitted the package, we provided SORN 09-20-0161, “Records of Health Professionals in Disease Prevention and Control Training Programs” as the applicable System of Records. We believed this SORN to be most applicable because CDD fellowships are training programs that prepare health professionals to prevent and control disease. We recently learned that SORN 09-20-0161 may no longer be an active System of Records, and simultaneously became aware of the existence of SORN 09-20-0112, "Fellowship Program and Guest Researcher Records" HHS/CDC/AHRC. We now recognize that SORN 09-20-0112, "Fellowship Program and Guest Researcher Records" HHS/CDC/AHRC is the more applicable System of Records, given our goal of allowing applicants (who include persons from various disciplines, including economics and informatics) to apply to CDC fellowships on-line and to track fellowship alumni in one integrated database.

Thus, we are requesting the change to OMB Package 0920-0765, Part A, Section A10, Page 5 to state the following: “The applicable System of Records is SORN 09-20-0112, Fellowship Program and Guest Researcher Records, HHS/CDC/AHRC.”

Please note that we have made the change in the attached document: OMB Package 0920-0765, Part A, Section A10, Page 5. There are no additional changes to this OMB package.
