Instructions: This survey is not for response by individuals. The survey and all responses are representative of the
school, line office, or ADD office as a whole. All survey questions must be answered in order to complete the survey.
Please choose one person from your school, line office, or ADD office to be the designated respondent for this survey.
The respondent will be responsible for ensuring that all questions are answered accurately and completely on behalf of
the school, line office, or ADD office. Please note that it may be necessary for a respondent to seek out, obtain, and
consult with additional experts in the school, line office, or ADD office in order to fully complete all questions in the
survey. Only one survey may be submitted for each school, line office, or ADD office. It is highly recommended that the
respondent review the survey prior to completion to ensure that they have all of the information necessary to complete the
survey. Once a respondent starts the survey, each question must be answered in order to proceed to the next question or
section. Incomplete surveys will not be accepted nor will the respondent be able to submit an incomplete survey.
Respondents may submit the survey more than once, if for any reason they feel that they misrepresented the school, line
office, or ADD office, however, in this instance, a respondent must also notify Brandi Sweet, Bureau of Indian Education,
[email protected], requesting to remove the misrepresented survey from the survey queue.

what training areas need to be addressed at the 2012 BIE Summer Institute. Public reporting burden for this collection is
