No information collection instruments seeking to collect information beyond the scope of customer satisfaction data, such as social-science or visitor-use information will be considered under the scope of this Programmatic Clearance. The submissions for each instrument must include the following information: 1. Which program office will be conducting this survey? Who are the customers? Which of the six topic areas will be addressed? 2. How was the survey developed? Who was consulted? What pre- testing, if any, was done? 3. How will the survey be conducted? How is the sample selected? What is the expected response rate and what steps are taken to maximize it? Which mode will used to administer the survey? 4. How will the results of the survey be analyzed and reported? If the customer population will be sampled, what statistical techniques will be used to generalize the results to the entire population? What level of aggregation will be used? At the conclusion of the review, OMB will inform the OPPM contact by email when the survey has been approved, after which the agency may use OMB number 1040-0001 on the instrument.
