FSIS 9540-1, IMPORT INSPECTION APPLICATION
(Meat, Poultry & Egg Products)
The following is guidance for applicants/custom brokers importing meat, poultry or egg product to the United States. The FSIS form 9540-1 has been modified to capture
more detailed information about shipments.
Block:
1. Country of Origin. This is the country that issued the inspection certificate for the export of the product.
2. Inspection Certificate Number. The serial number, including dashes/slashes, of the foreign country certificate covering the product in a shipment.
3. Exporting Establishment Number. This is the official number of the foreign facility that the product was exported from and contained on the inspection certificate.
4. U. S. Point of Entry/CBP Port Code. This is the name of the port of entry1 and the associated CBP port code designation where the entry is filed with U.S. Customs
and Border Protection (CBP). For example, if the entry is filed at the Port of Philadelphia for merchandise unlade at Long Beach, CA, enter "Philadelphia, PA - 1101"
as the U.S. Port of Entry and Port Code.
5. U. S. Point of Unlading/CBP Port Code. This is the name of the U.S. port of entry1 and the associated CBP port code designation at which the merchandise was
unladed. Examples: 1) When merchandise is taken off a vessel in Long Beach, CA, and entry is filed at the Port of Philadelphia with CBP, show "Los Angeles/Long
Beach, CA - 1104" as the port of unlading; and, 2) For shipments entering from Canada, the port of unlading is the port at which the import establishment is located
(e.g., Champlain, NY - 0712).
6. Import Est./Egg Breaking Est. No. This is the official number assigned by FSIS to an official import inspection establishment or the number assigned to an approved
egg breaking establishment where the product will move for inspection or breaking.
7. Name and Address of FSIS Import Est./Egg Breaking Est. The name and address of the official import inspection establishment where the product is to be moved for
FSIS import inspection or the name and address of the approved egg product plant where the eggs will be moved for breaking.
8. Name and Address of Customs Broker or Applicant, as applicable, including zip code. Self explanatory.
8a. Reference Number. A number assigned by the customs broker or applicant for their internal tracking of the shipment.
8b - 8e. Self explanatory.
9. Name and Address of Consignee, including zip code. The name and address, including zip code, of the company or person to which the product is consigned.
10. Importer of Record Number. This is the importer's Internal Revenue Service (IRS) business registration number. If the importer's business is not registered with the
IRS, either the social security number of the importer or the CBP assigned importer number for the importer entered in block 11 shall be sufficient.
11. Name and Address of Importer of Record (IR), including zip code. The name and address of the Importer of Record as declared to Customs when entry is filed.
12. Lot No. The lot number of each product identified on the foreign inspection certificate. Start with 1 and continue with 2, 3, etc.
NOTE: Blocks 13 through 33 repeat for each lot on an inspection certificate.
13. Shipping Mark. Enter the unique number assigned to this lot of product that provides traceability to the inspection certificate and is applied to the outside containers.
Include slashes/dashes as appropriate.
14. Custom Entry Number(s). Enter each applicable 11 digit custom entry number(s) associated with the lot. First three characters are alphanumeric; last eight are
numeric; no slashes or dashes. When more than one custom entry number, separate by comma, but no space. Limit to seven custom entry numbers per lot. For
American Samoa, Guam, and Saipan, enter enough zeros before the custom entry number to make an eleven digit number.
15. Production Date(s). Enter the range of production dates for the product in the lot. This may be slaughter dates, production dates, sell by dates, best if used by
dates, etc.
16. Net Weight of Lot. Enter the net weight of the lot in pounds from the inspection certificate.
17. Shipping Unit Package Type Name. Enter the type of shipping unit packaging (e.g., box, carton, combo, tote, etc.).
18. Number of Units. Enter the number of shipping units from the inspection certificate.
19. Immediate Unit Package Type Name. Enter the type of immediate container packaging (e.g., can, jar, trays, plastic wrap, etc.).
20. Number Per Shipping Unit. Enter the number of immediate containers contained in a shipping unit. When the amount is not standard in each shipping container,
enter an average/approximate number.
21. Seal Number(s). Enter the conveyance seal number, when appropriate.
22. Processing Est. No. The foreign establishment number of the foreign plant that produced the product (e.g., last processed the product).
23. Source Country(s). List each country from which product was sourced to produce the product in this lot.
24. Source Est. No. List each establishment from which product was sourced to produce the product in this lot.
25. HTS Code. The Harmonized Tariff System2 code for the product contained in this lot. The length of the code entered (6, 8, or 10 digits) shall be sufficient to
properly identify the product. Do not use periods/decimal points in the code.
26. Process Category. For meat and poultry products, enter the applicable process category form 9 CFR 417.2(b) that the product was produced under.
NOTE: "Slaughter" is not used for imported products. For eggs and egg products, enter "Eggs/Egg Products".
27. Product Category. Enter the most appropriate product category from FSIS Directive 9900.2, Annex 3.
28. Product Group. Enter the most appropriate product group from FSIS Directive 9900.2, Annex 3.
29. Species (dominant). Enter one of the following, as applicable: Beef, Veal, Goat, Lamb, Mutton, Pork, Horse, Equine other than Horse, Chicken, Duck, Goose,
Guinea, Squab, Turkey, Emu, Ostrich, or Rhea.
29a. Additional Species (if applicable). Where the product is made from more than one species, enter the appropriate additional species from 29.
30. Description of the Product. Enter the name or description of the product. For example, lamb legs, beef short loin, etc.
31. Supplemental Product Code. Enter the UPC, GTIN, or other product code that is used in commerce for the product..
32. Bill of Lading Number(s). List each number associated with the product in the lot.
33. Estimated Date of Arrival. The date that the product is expected to arrive at the official import inspection establishment or approved egg breaking establishment.
