The Centers for Disease Control and
Prevention proposes to conduct continual national evaluation of the
agency's Consumer Response Services Center--CDC-INFO. CDC-INFO is a
proactive, unified, and integrated approach to the delivery of
public health information. CDC-INFO is designed to contribute to
improving the health and safety of the public. Customers are
defined as any individual or group seeking health or public health
information from CDC. This includes the businesses, researchers,
and others. Customer interactions occur through multiple channels,
e.g., telephone calls, e-mails, and postal mail and may occur in
Spanish or English. CDC requests the reintroduction of the live
phone follow-up survey, previously deleted due to budget
constraints, as part of the Evaluation of the Centers for Disease
Control and Prevention's National Contact Center
(CDC-INFO).

This change would reinstate
CDC-INFO's Live Phone Follow-up Survey to the current OMB package,
revised and approved in 2010. The Live Phone Follow-up Survey was
originally included and approved in the first CDC-INFO Information
Collection Request in 2007. No changes have been made to the survey
since it was first approved. The estimated number of persons who
participated in this survey each year was 3,125. The survey length
was 7 minutes, with a yearly burden of 365 hours.

the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
