This information collection requires the owner or operator of a vessel, within 5 days, to report a marine casualty. Additionally, the owner or operator of a vessel must periodically report chemical drug and alcohol testing of commercial vessel personnel. |

When must information be submitted to the Coast Guard? |
For reporting a marine casualty, within 5 days of the casualty. For chemical drug and alcohol testing of commercial vessel personnel, periodically according to the requirements in 46 CFR Part 16. |

All the above information may be submitted in writing or electronically via facsimile or e-mail. |
What happens when complete information is received? |
This information will be reviewed by CG investigators. No specific documentation will be issued by the CG. |
