ABOUT OUR SURVEY
Social Security would like to know how well we served you when you filed your recent application for
retirement benefits. The survey asks you to rate the service you received in person, on the telephone, or on
our website. This survey also asks about your expectations for good service and how you want to do business
with Social Security in the future. The survey should take about 20 minutes to finish.
